The chart below maps out where the notes from the open strings to the twelfth fret are on the guitar. Notice the notes at the twelfth fret are the same as those of the open strings.

As an example using this chart, let's say you want to know which note is at the fourth string, third fret. Count from right to left four strings. This is the D string. Next, follow the numbers down the left side until you find the number of the fret you want, in this case three. Now follow the D string down until you are next to the three. The note at this location is an F. Therefore, the note on the guitar at the fourth string third fret is an F note.

Beyond the twelfth fret the notes repeat again. The twelfth fret notes are the same as the open string notes only an octave higher in pitch. The thirteenth fret notes are the same as the first fret notes, once again, an octave higher in pitch, and so.
